Office of Faculty Affairs responsible for issues relating to School of Medicine faculty, including 1.Recruitment, promotion, and tenure 2.Faculty orientation, counselling, and mediation 3.Faculty development and governance 4.Grievance procedures, contractual, compensation, legal issues 5.Provide Faculty Handbook, maintain a Faculty database, and work with other committees to develop new policies

Functions Faculty performance evaluation Serve as a liaison to the office of the dean, the office of the provost, the administration and the faculty

Annual Faculty Performance Evaluation required for all faculty at WUSM Aim: to provide i.a retrospective evaluation ii.an agreement on shared goals for the future to benefit both faculty member and the school

Performance Evaluation document faculty member’s activities/accomplishments provide feedback (positive + areas of improvement) define the goals for next year define a plan for academic/professional development

Inability to achieve the desired outcomes without adequate alternative accomplishments may require a change in direction/effort for the faculty member

Distribution of effort among faculty members 1.Some with more teaching 2.Some with a greater involvement in research 3.Some with more administrative service

Evaluators must provide clear goals for teaching and for research on this evaluation

Teaching Activities limited to teaching activities during the period of July 1, 2014 through Jun 30, 2015 (Annual Review)

Teaching Accomplishments 1.Teaching activities 2.Role/function in the activity 3.Name of the activity 4.Level of individuals being taught 5.Number of individuals participating in the activity 6.Frequency /time of the activity 7.Evaluation scores, if available 8.Mentoring of students 9.Teaching awards and honors

Self-Assessment of Teaching Activities 1.Extent to which the faculty met teaching goals 2.Challenges faced during this academic period, and the approach taken to address each challenge 3.What the faculty, the department, or the institution could do to enhance professional development in this academic area

Evaluation of Teaching Activities 1.Comments on accomplishments, strengths, weaknesses, and if the annual performance goals were met 2.Detailed comments and specific expectations when improvements are needed
