To ensure a continuum of care for patients with brain injury. To formalize the process of referral to BIAC and promote seamless community reintegration. To remove burden off of patient/family to find us. Who is BIAC? The go-to resource for help and services for survivors of an injury to the brain, their family, and providers. Resource navigation, case management, support groups, workshops/classes, educational materials, adaptive recreation, and more. Why the Partnership?

Patient is presented with cover letter and release form. If signed, send to medical records who will fax us the appropriate documents. BIAC will contact patient and establish a plan for ongoing follow up. Who to Refer? Anyone with a brain injury Examples: Stroke, Anoxia, Traumatic Brain Injury, Infection, Brain Tumor How to Refer?
