Scanned copy of any of following Age Proof Document should be in jpg Format and Max Size of PDF should be 500kb only. a) Birth Certificate issued by a Municipal Authorities or District Office of the Register of Births & Deaths or Baptism Certificate; or b) Birth Certificate form the school (Govt. /Recognized) last attended by the applicant or any other recognized educational institution’ or c) Marksheet of class 8/Class5 or Class 10, if it contains date of birth; or 3. Scanned copy of any of following for Address Proof Document should be in jpg Format and Max Size of PDF should be 500kb only a) Bank/Kisan /Postoffice current Pass Book or b) Applicant ‘s Ration Card/Passport/Driving License/Income Tax Assessment Order or c) Latest Water/Telephone/Electricity/Gas Connection Bill for that address either in the name of the applicant or that of his/her immediate relation like parents etc.or d) Postal Department’s posts received/delivered in the applicant’s name at the given address.
