Patient Wellness Handout

Institute of Medicine  The Institute of Medicine (IOM) released “To Err is Human” and “Crossing the Quality Chasm” which identify safety and quality problems that exist within our health care system.  These reports identify and define six quality aims: Safety Effectiveness Patient-centeredness Timeliness Efficiency Equity

Patient Centeredness  Patient-centeredness is respectful and responsive care provided to individual patient preferences, needs, and values and ensures that patient values guide all clinical decisions.

Ten Needs Required by Consumers 1.Care based on healing relationships 2.Customization based on patients needs and values 3.The patient as the source of control 4.Shared knowledge and the free flow of information 5.Evidence-based decision- making 6. Safety as a system property 7. The need for transparency 8. Anticipation of needs 9. Continuous decrease in waste 10. Cooperation among clinicians

Put Prevention into Practice  The Agency for Healthcare Research and Quality (AHRQ) Put Prevention into Practice Designed to improve the delivery of clinical preventive services to patients through the use of tools that provide the health care consumer with knowledge and empowerment.

Put Prevention into Practice  Inter-collaborative process between patients and providers to: Determine which services their patients should receive. Facilitate the delivery of clinical preventive services. Make it easier for patients to understand and keep track of their preventive care  Provide the patient with appropriate health care literature and information  Encourages them to take a proactive and motivated approach to managing their health and well being in cooperation with health care professionals.

Patient Wellness Handout  The IHS Patient Wellness Handout combines features of the RPMS Health Summary with the concepts developed by the Agency of Healthcare Research and Quality's (AHRQ) Putting Prevention Into Practice program.  These features attempt to address the Institute of Medicine's (IOM) 10 rules of patient centered care and empower patients to improve their health and satisfaction with medical services.

Patient Wellness Handout  Provides access to information in their medical record. Immunizations due Weight, Height, BMI Blood pressure Allergies, Current medications.

Patient Wellness Handout  In combination with patient education, the patient medical handout attempts to provide the tools a patient requires to improve collaboration with providers and to assist them in make appropriate health care decisions.

Patient Wellness Handout v1.0  The first version of the PWH was released on September 1, 2006 as a mandatory install to all IHS service units.

Who can generate a PWH?  The PWH can be generated by data registration, clinicians, pharmacists, or anyone else who has normal access to the patient's medical record.
