JABSOM Annual Faculty Reporting Instructions For Reporting Session: July 1 – June 30
Notes All reporting is for academic year (July 1 – June 30) Reporting is required for all compensated faculty only Faculty with joint appointments should complete a report for your primary department only You will need a UH username/password to log in All text comments need to be kept at a 500 word limit Upon submission deadline, forms will not be accessible as to allow for your chair to begin reviewing all entries.

To Begin Your Reporting Session Go to the Faculty Development Website: 1. Click on the “Annual Faculty Reports” tab 2. Click on “Annual Report Forms – All Faculty and Chairs – Log in Here”
Log-In Screen You will then see the log-in screen. Log-in with your UH username and password (same as your UH account)Log-in with your UH username and password (same as your UH account)
Reminders Click on the “save” button at the end of each section to save your work At the end of your session, you must click on the “submit” button to submit your form You may submit/change your data as many times until the deadline Upon clicking “submit” a printable version will appear Click on “Click to Print” button to print a copy for your files Click on “log out” to end your session If you need to make changes after submitting your report, please contact Lori Emery at or to re-open your report
Reporting Forms You will have two forms to submit: 1.Annual Faculty Report 2.Evaluation of Department Chair Survey

Annual Faculty Report Your contribution to your department and JABSOM’s annual report Used for annual feedback and to monitor progression towards applying for promotion and/or tenure Used towards UH annual faculty contract renewal process (as applicable) Be as accurate as possible When unsure, please make your best estimate Each year’s report will be stored for future reference

Annual Faculty Report: Process Upon submission deadline, your Department Chair will review your report and include comments at the end of the form Your chair should schedule a one-on-one meeting with you to discuss your efforts and goals for that year You will both sign the form at time of meeting and your department should keep it on file
2. Evaluation of Chair Form This form allows faculty to rate their chair anonymously anonymously Please evaluate only your department chair chair The origin of the report will not be disclosed to your chair Your name must appear on the form for validation purposes only validation purposes only
2. Evaluation of Chair: Notes After the deadline, a summary of chair evaluations by department will be reviewed by the Dean. The Dean will meet with all Chairs one-on-one to discuss overall outcomes of the evaluations
