 Community of interest: Senior residents aged 65 years old or more  Assessment method: Progressive inquiry questions, key informant interview, pilot test and survey questionnaire using door-to-door approach.  Assessment findings: 13 different languages identified, high cardiovascular diseases, and low community activity participation

 Community Of interest: New comers (English as a second language adult students)  Assessment Methods Used: Secondary Data collection, Primary Data collection(Mapping Of area, key informants, Progressive Inquiry, group discussions)  Assessment findings: Opportunity to expand health literacy, health promotion strategies and improve knowledge of health care practices  3 Common practices covered: Hand Hygiene & Flu, hypertension, and Cancer
