Survey List Page  List of surveys will appear along with due date and expiration date (in ascending order)  Participants will be unable to open ‘Completed’ or ‘Expired’ surveys

Survey Dates for Site Coordinators  Demographics and Outcomes Survey  Open date – start of Phase II  Due date – Initial Survey, at 12-Month Survey, and Final Survey  Expiration date – 3 months past due date

Survey Dates for Study Participants  Open date – date when survey is made available to user  Due date – date when survey is due for completion  Expiration date – grace period for survey completion; survey is no longer available after this date
