Co-ordinating Care Good co-ordination can reduce health service inefficiencies and provide a better patient experience Lack of co-ordination of care across and between the different disease pathways and service providers This was a factor that made people more vulnerable to premature death

Problems Systemic problems with the organisation of services Lack of holistic care Davids story Emphasis on episodic rather than on-going care Lack of awareness of others roles

Good practice Good communication is essential for well co- ordinated care Susans story CLDTs and LD liaison nurses can help with care co-ordination

Recommendations Co-ordination of care role enshrined and monitored in contracts across health and social care Named lead professional to take the healthcare coordinator role Co-locating CLDT nurses into primary care
