VUHID Update for CHC Collaborative Health Consortium Barry R. Hieb, MD Chief Scientist, Global Patient Identifiers Inc. Updated Dec., 2011 \marketing\presentations\CHCpresentation

Accurate Patient Identification is Essential Clinical Care –Right treatment for the wrong patient causes harm Cost savings –Duplicate records, lost information, wasted time Information exchange –Comprehensive records, public health Privacy –Consents, legal compliance Fraud & abuse –Need a mechanism to recover from errors 2

Unique Identifiers are the Optimum Patient ID Strategy Simple Permanent, but replaceable if necessary Concise, can be incorporated into data sets Cost-effective Globally unique Flexible, works with variety of systems & methods Available Capacity Directly involves the patient Minimal impact on existing systems 3

Current Patient Identification Infrastructure is Inadequate EMPI matching 8% to 12% error rate Many known problems –Duplicate names –Siblings/twins –Misspellings –Marriage –Foreign names –Large populations Over 100,000 deaths annually due to errors in patient identification 4

Identification Errors are a Patient Safety Issue False positives create a clear patient danger False negatives mean duplicate records and needed information may not be available Additional (duplicate) testing is expensive and can be dangerous Good clinical decision support requires accurate and comprehensive information Time spent looking for information is time not spent diagnosing and treating the patient 5

A Typical ID Card Using a VUHID Identifier ID card issued by HIE when patient enrolls - globally unique ID Identifier remains with the patient for life HIE and care delivery organization branding encouraged Card represents the ‘face’ of the HIE to the patient – Patient is reminded of HIE value each time they use the card Card has at least one machine-readable representation of the ID WEDI-compatible John Q Public Open Identifier Hypothetical Healthcare Inc. 6

7 Privacy Requirements Mandate Two Types of Identifiers VUHID identifiers are either Open or Private Open Voluntary Identifier – OVID –Patient identity is known –Information is available to all of a patient’s caregivers –Clinical, administrative, financial and other uses Private Voluntary Identifier – PVID –Many different classes –Patient identity is anonymous –Only three entities know the patient’s identity Patient, selected physician(s), HIE EMPI system –Specific set of “rules” for each privacy class An individual should have one OVID but may have many PVIDs

VUHID Identifier Syntax PrefixDelimiter Check digits Privacy class Identifier syntax John Q Public Open Identifier Hypothetical Healthcare Inc. 8

Privacy Considerations VUHID system is not a privacy threat –No names, demographics, clinical data Privacy classes represent the infrastructure for the ‘next generation’ of privacy management –Multiple concurrent models –Clinical data segementation –Evolution –Patient control –Complements ‘trust mechanisms’ e.g. DirectTrust –Leverage anonymity wherever feasible/desirable 9
