Arizona Early Intervention Program IDEA 2011 Requirements -Screening-

Screening If screening is conducted: Requires Prior Written Notice (PWN)NEW Requires ConsentNEW Includes the use of an appropriate instrumentNEW Allows Parent request for evaluation at any time during screening process, regardless of screening results. With consent, an evaluation must be conducted. NEW Requires Notice to parents after screening for child not suspected of having a delay to inform parent of ability to request an evaluation. NEW

Your partner for a stronger Arizona Screening processes are used for a child: Who does not have an established condition or other records to document eligibility of a 50% delay or When an appropriate screening tool was not submitted with the referral or For whom observation and parent report do not indicate a need to proceed directly to evaluation Screening

Your partner for a stronger Arizona At the initial visit, the service coordinator: Explains early intervention - Mission and Key Principles Provides Prior Written Notice- Explains purpose for screening and procedures, including the parent’s right to request an evaluation at any time during screening process Obtains Consent for screening using new form Screening

Your partner for a stronger Arizona At the initial visit, the service coordinator: Conducts a screening using an AzEIP approved screening instrument Completes the Vision Screening using the AzEIP Vision Screening Checklist Completes Hearing Screening Tracking form to ensure current hearing screening results are documented Screening

Your partner for a stronger Arizona After screening activities have been completed the service coordinator provides Prior Written Notice and explains results to parents. For children who are not suspected of having a delay in development, explain that AzEIP is not proceeding to an evaluation and the basis for that decision; parent may request an evaluation If parent requests evaluation, SC documents request in records and obtains consent to evaluate If evaluation is not requested during the screening process, SC closes the file For children who are suspected of having a delay in development, explain that AzEIP is proposing to proceed to an evaluation and the basis for that decision; obtain consent to evaluate if parent wishes to proceed Screening Process
