Theme: Success Supported by Measurement What are some of the significant changes, improvements, and/or innovations that you’ve realized by leveraging data and narrative information? Please describe one or two important change(s), improvement(s), and/or innovation(s) that your team has implemented to better serve your clients living with HIV. In your presentation, please describe the change and how you used data and/or narrative to support it. Share any key lessons too! Think about this broadly and be creative! Use data, pictures, and anything that helps to tell your story!

More guidance Here are some examples of how teams might have used data, narrative and other artifacts to support their changes: We have built strong relationships with our peers and this helps us to improve our services at a faster pace; we found our shared indicators helped us to compare what we were doing and find common ground when communicating. We used run charts to engage our leadership and support our work We used stories to coordinate and collaborate with community agencies We used our data to communicate with new team members We have sustained our client feedback surveys and continue to use this information to build better systems and communicate with our clients Things to consider including: Focus on describing the important change(s), improvement(s) and/or innovations that your team implemented to better serve your clients living with HIV. Why it is important? How did it come about? How did it further your improvement agenda? Describe the role that data, narrative, and/or other artifacts that you collected supported you in this making this important change. Lessons learned which could be applied to other teams/sites. Anything else you feel is important to be shared in the Network.
