A recent industry study by the Health Information Network identified five key challenges in implementing palliative care into transitions programs. We will address each and give each panelist a chance to address each in this panel.

51.6% of respondents identified timely referrals as a challenge in implementing palliative care in transition programs.  Do you agree that this is the biggest challenge?  What are a few strategies to improve this problem?

48.4% of respondents identified Physician Resistance as a challenge in implementing palliative care in transition programs.  Do you agree that this is one of the biggest challenges?  What are a few strategies to improve this problem?

46.2% of respondents identified resource allocation as a challenge in implementing palliative care in transition programs.  Do you agree that this is one of the biggest challenges?  What are a few strategies to improve this problem?

33.0% of respondents identified reimbursement as a challenge in implementing palliative care in transition programs.  Do you agree that this is one of the biggest challenges?  What are a few strategies to improve this problem?

29.7% of respondents identified patient buy-in as a challenge in implementing palliative care in transition programs.  Do you agree that this is one of the biggest challenges?  What are a few strategies to improve this problem?

Why is it such a challenge for primary care physicians and palliative care specialists to work together?
