Tier I Quarterly Submission Hospital A 2Q, 3Q & 4Q Submits same measures with all three months completed by July 31 Receives $250 each quarter Total Possible: $1750 Hospital B 2Q, 3Q & 4Q Submits same measures with all three months completed by July 31 Receives $500 for each quarter Total Possible: $3500

Tier II & III Tier II Submits at least one HAB report for each quarter $250/quarter ($1000 possible) Tier III Achieves 40/20% reduction from baseline in applicable topics by Nov 1 OR sustains rate at or below benchmark $1000 incentive
