10 PFC Eligibility Guidelines Applicants must have: Implemented a source Reduction, Reuse, and/or Recycling program whereby at least 10% of total waste has been recycled, reduced or reused. Have the following policies in place and be implementing the following plans/programs at some level, but full implementation may not have been necessarily been achieved. mercury elimination program waste management plan that includes a waste reduction and pollution prevention policy regulated medical waste reduction and management program

11 ELA Eligibility Guidelines Applicants must have… Met the Making Medicine Mercury Free award criteria. A 25% recycle, reduce or reuse rate A waste management plan that includes a waste reduction and pollution prevention policy. A regulated medical waste reduction and management program in place. A “environmentally preferable purchasing policy” in place. A long-term, sustainable environmental programs that have become institutionalized that can serve as model practices for the industry. Innovative programs, techniques, processes and policies that increase environmental performance. Efforts might include collaboration with the community, educational programs and cost accounting.

12 Partner Recognition Designed to encourage hospitals that are new to program Hospitals must meet self-set goals for the year to receive award Given to all hospitals that fulfill criteria No ceremonies are planned for Partners receiving recognition

13 Champions for Change Award Designed to recognize Champions for helping to promote H2E Must describe and document their activities in promoting H2E Given to a select handful of Champions

14 How often can I apply? Annual Awards Applications accepted from Dec-Feb H2E Environmental Leadership Award –H2E Sustained Environmental Leadership Award H2E Partners for Change Award H2E Recognition One Time Award Making Medicine Mercury Free Award Applications accepted throughout the year!

15 Deadline H2E is accepting award applications from December 1, 2004 until February 15, All applications must be received by close of business on February 15th. This deadline is for both ed electronic files and mailed hard copies. Submittal Process an MS Word copy of the application by February 15, 2005 to Include as many supporting documents as possible in an electronic format. Please use your facility name in the subject line. Jolie Patterson Rosst will reply to this to confirm receipt of your application. If she does not reply within 1 business day please either re-send your application or call to notify H2E staff that you are having a technical problem. Submit by mail 3 copies of your application with all supporting documentation including documentation that you were not able to include in your version. The mailing address can be found on the application cover page. Important Details to Note Incomplete or late award applications will not be considered for this award cycle. No supplemental materials will be allowed after February 15, Award application materials become the property of H2E -- they will not be returned. All applications require supplemental documents to support the information you present. Please limit these materials to no more than 10 pages for the Partner Recognition application and 15 pages for all other applications. Supporting documentation refers to sample policies, training programs, tracking forms, and other information that could serve as a model for other Partners. We discourage the use of electronic presentations and meeting minutes as supporting documentation. How to Apply – for all awards

17 Can I apply for more than one award? YES… but Each award is designed to recognize healthcare facilities at a different level of environmental achievement. Refer to the award criteria for each award to determine which one best fit your facility’s accomplishment to date. You may apply for the Making Medicine Mercury Free Award along with the H2E Partners for Change Award -- provide complete application forms and materials for each award. The applications and the supporting materials are reviewed by different panels.Making Medicine Mercury Free AwardH2E Partners for Change Award H2E Environmental Leadership Award applicants must also meet the MMMF award criteria so do not apply for both awards. Winners of the ELC Award get 2 award plaques. H2E Environmental Leadership Award

18 Can I be considered for an award that I did not apply for? Yes but under special circumstances. if you submitted an application for the H2E Environmental Leadership Award but did not necessarily meet all the criteria for that award, you still may be eligible for a H2E Partners for Change Award or a Making Medicine Mercury Free Award. 19 More FAQs Can I apply for the Mercury Award if I have not met all of the criteria? No, applying for the award essentially assumes you have met all of the criteria. If you have not met the criteria, keep working on your program and apply when you’re there. What supporting materials should I include? Each award application must include the award cover sheet, the specific awards checklist and any supporting documentation to confirm the work. Also, we invite you to include examples of innovative work that can be shared with other facilities. Can I apply for a Partner Award as a health system? We encourage individual facilities to apply for awards separately. Can I apply for an award if I have RCRA, EPA, OSHA or other environmentally related violations? Please contact the H2E program if you’ve had past violations. Ongoing or current enforcement actions may preclude you from applying.the H2E program

25 MMMF and Partner for Change Award Winner “Hartford Hospital has received the Partner’s for Change Award in 2002, 2003 and 2004 as well as the Making Medicine Mercury Free Award in The award process has helped the institution more closely gage what has been happening with our environmental management system over this time period and has allowed the hospital to set realistic goals to improve our waste amounts and bottom line.”

26 MMMF and Partner for Change Award Winner Over the past 6 years, Hartford Hospital was able to reduce hazardous waste amounts by over 70% and medical waste 50%. These levels have been either remaining constant or decreasing as the institution continues its waste minimization efforts.
