In 2010 the Northwest Portland Area Indian Health Board received funding to assist NW Tribes with Public Health accreditation and Quality Improvement. At the time we were in the midst of redesigning our clinical system of care. We were also reorganizing in other areas, such as Community Health Services.

1.How do we improve PH Dept. to meet the needs of our population? 2.Are the services/programs and our workforce optimal for improving PH? 3.What are our PH roles/authority given State and County PH entities?

Activities to improve PH Dept. and Health Status:  Community Health Assessment (CHA)  Community Health Improvement Plan (CHIP)  Agency Strategic Plan  Workforce Competencies  PH Law and Code Review

Process: Collaboration with St. Anthony’s Hospital. Umatilla County Community Partnership. Community Health Partnership Journey.

Process: Adopt Healthy People 2020 Objectives; incorporate into Annual Work Plan.  Holds us to a higher standard and it is measurable. Requires community participation; Public Health Accreditation Team (PHAT). Supports development of a QuaIity Improvement program.

Foundation of Strategic Plan: Needs assessment: currently asking the community what services they need and/or want. Other assessments: Planning Committee, Circles of Care, and Maternal Child Health.

Position description assessment (Mills Group NW). Assessment of competencies. Do we have the right positions given our user population’s health needs? Identify training to develop Public Health capacity within workforce.

State Laws County Roles Tribal Code Federal Laws

CDC consultation helped identify deficiencies; helped define roles and responsibilities. Found many Tribal Laws and Codes need to be updated. Need to partner with Community Health Services, Pharmacy, and others. MOUs with Umatilla County.

Starting from where we were. Need to take one step at a time. Public Health Accreditation is a roadmap. Shifts in organizational culture.

Educating staff and leadership. Readiness checklist; working through Self- Assessment Workbook for Tribal PH Depts. Completion of CHA; drafts of CHIP and Agency Strategic plan. Workforce: position description evaluations; beginning competency assessments. PH Laws: work-in-progress; better understanding of roles/responsibilities within legal landscape.
