SAFETY NET INNOVATION NETWORK Moving Forward - July 2012

Support the adoption and spread of innovations to improve care delivery with the aims of better health, improved patient experience and lowered costs in the Safety Net in California GOAL of Innovation Network

Key Objectives 1.Create a network of safety net leaders with strong relationships to PARTNER on projects 2.Create a forum for discussions about the SUCCESSES and FAILURES in implementing innovations and an evaluation framework to assess why and how innovations succeed 3.Build SKILLS and capacity to use new TOOLS for innovation and creating a culture of innovation 4.Create RESOURCES to help identify, assess and IMPLEMENT a range of innovations 5.PILOT and SPREAD compelling innovations across safety net organizations with technical assistance and funding 6.As appropriate, identify areas where the safety net could ACT AS A WHOLE to remove barriers for innovation

Roadmap for Network Identify 3 areas for focused innovation related to triple aim Identify technical assistance providers Apply innovation methodologies Developing a culture of Innovation Innovation skills including observations, critique, storyboarding Content related to specific innovations Relationships Skill Building Action What How Technical experts in specific areas made available Modest funding to support roll out & evaluation Site visits & include non health care Foster a network - start with those who attended meeting Create application process to encourage broader involvement over time Twice/year meetings Site visits Use video, phone, web to communicate Leverage existing web platforms (Linked In) Connect members to other opportunities T rainings connected to bi-annual meetings Webinar coaching Package skills training as needed Connect people to external training offerings

Initial FOCUS Areas for Innovation* 1.COST: Coordination and Communication after hospital/ED discharge 2.PATIENT EXPERIENCE: Reduce wait times for primary care 3.BETTER HEALTH: Effectively using all members of care teams to manage populations *NOTE: Focus areas were determined based upon SNIN kick-off meeting and stakeholder interviews. Topics will act as anchor for innovation activities in first year & push thinking
