Goals of our monthly webinars Our goals for our MFP monthly webinars are:  To provide training on key topics  To better the lines of communication between you and us, the MFP staff  To seek feedback on how to better the processes of MFP/waiver services for our participants  To provide the opportunity to share successes and challenges as RTC’s and MFP transitions

MFP Transition Numbers As of today July 9, 2015  131 referrals have been received  37 participants have transitioned to date (15 in 2014 and 22 thus far in 2015) 4-6 transitions set to go in July

MFP Guidance for Participant Placement In Assisted Living Facilities (ALFs) The guidance was designed to assist RTC’s in understanding MFP’s expectations surrounding ALF placements by addressing the following concerns:  Concerns from RTC’s about MFP not allowing participants to transition into ALF’s that don’t currently meet the HCBS Settings Rule  Concerns from Waiver partners that MFP participants are not being informed of the risks of moving into a ALF that may not meet the rule in future  Concerns that MFP participants are not being informed of other community housing options Assisted Living Participant Selection Form

Successful Transition Story  Susie Goffena with Area II Office on Aging - Roundup Medicaid Waiver served as the RTC.  Participant transitioned from a nursing home to a Billings subsidized housing apartment in March 2015.
