GOAL #1 OUTREACH Enrollment via engagement of Veterans in their community or surrounding areas. Registration – assist with the completion of form 1010 for healthcare. Primary Care and Mental Health service connection. Provide additional information and assistance on form (with the support of VSO offices). Assist in the navigation of the Department of Veterans Affairs system……or at least refer to the appropriate department. Reach out to as many Veterans in Rural American as is possible. 10

GOAL #2 EDUCATIONAL ENHANCEMENT Increase the veteran’s knowledge of, compliance with and awareness of chronic diseases such as Cardiovascular Disease (CVD), Congestive Heart Failure (CHF)and Diabetes Mellitus (DM). Present and provide education and guidance on healthcare information that is geared towards the 9 healthy choices as defined by VHA National Center for Health and Disease Prevention. 12

GOAL #3 COMMUNITY PARTNERSHIPS Identify and partner with community organizations to create forums in the rural areas for health education classes, programs and outreach. Identify and partner with community leaders to ensure that Veterans have every opportunity to maximize their health outcomes through VHA, VBA and NCA services. 14
