Health & Wellbeing in Our Communities

Welcome Elidh Myrvang Brown, Third Sector Development Officer, TSI Moray Robin Paterson, Integration Project Officer, The Moray Council

What we’ll be talking about Health and Social Care integration What you want to see in the draft strategic Plan for your community Funding opportunities for community initiatives Your questions

What is integration? At its heart, health and social care integration is about ensuring that those who use services get the right care and support whatever their needs, at any point in their care journey… and it’s also about helping to build stronger, healthier communities.

The Public Bodies (Joint Working) Scotland Act 2014 Key elements of the Act… Partnerships will be measured against 9 high level national health and wellbeing outcomes. Partnership will have a Partnership Board and a Chief Officer. The Local Authority and Health Board will make payment to a joint budget overseen by a Integration Joint Board.

The Public Bodies (Joint Working) Scotland Act 2014 Key elements of the Act Partnerships are required to undertake locality planning and segment their boundaries into at least 2 geographical areas. Partnerships have submitted an Integration Scheme to the Scottish Government by the deadline of 1 April 2015 and are now working on their Strategic Plans.

The journey to integration Scottish Government passes the Public Bodies (Joint Working) Scotland Act 2014 Integration Scheme document for Moray produced and consulted on Shadow Integrated Joint Board formed Chief Officer appointed Strategic Planning Group established Draft Strategic Commissioning Plan being developed

The National Health and Well being Outcomes Proposed 1Healthier Living People are able to look after and improve their own health and wellbeing and live in good health for longer 2 Independent Living People, including those with disabilities, long term conditions, or who are frail, are able to live, as far as reasonably practicable, independently and at home or in a homely setting in their community 3Positive Experiences People who use health and social care services have positive experiences of those services and have their dignity respected. 4Quality of Life Health and social care services are centred on helping to maintain or improve the quality of life of service users.

The National Health and Well being Outcomes Proposed 5Addressing Health Inequalities Health and social care services contribute to reducing health inequalities. 6Carers are supported People who provide unpaid care are supported to reduce the potential impact of their caring role on their own health and wellbeing. 7Services are safe People who use health and social care services are safe from harm. 8Engaged workforce People who work in health and social care services are supported to continuously improve the information, support, care and treatment they provide and feel engaged with the work they do. 9Effective resource use Resources are used effectively in the provision of health and social care services, without waste.

Our Vision Statement Our Vision “To enable the people of Moray to lead independent, healthy and fulfilling lives in active and inclusive communities, where everyone is valued, respected and supported to achieve their own goals.” Shadow Integrated Joint Board

Local priorities Based on what we know about: Our population and their health and wellbeing needs The services we currently have The pressures already on services What should our local priorities be?

Getting the Strategic Plan right for Moray 1.What do health and social care do well? 2.What needs to be better? 3.What community activities already contribute to improving the health and wellbeing of people in your area? 4.What would help you and your community to do more? 5.What are the two top priorities you want to see in the plan and why?
