What is Integration for? “Health and social care services should focus on the needs of the individual to promote their health and wellbeing, and in particular, to enable people to live healthier lives in their community” (SG, Outcomes Framework, overarching statement)

What is Integration for? (2) “Our vision is that by 2020 everyone is able to live longer healthier lives at home or in a homely setting” (NHS Scotland Route Map to 2020) “Our vision is that people will lead longer and healthier lives by getting the right advice, care and support, in the right place, at the right time” (Midlothian Partnership)

How will integration achieve this? Integrated services and budgets – to enable prioritisation Inclusion of selected housing services Strategic Planning Locality Planning Coordination with other agencies including housing services

The Outcomes Framework To measure the progress we are making To ensure we keep heading in the right direction To keep the experience of the individual at the centre

The National Health & Wellbeing Outcomes People are able to look after and improve their own health People are able to live independently and at home or in a homely setting People who use services have positive experiences, and have their dignity respected Services are centred on the quality of life of people who use the services

The National Health & Wellbeing Outcomes (continued) Services contribute to reducing health inequalities Carers are supported to look after their own health and wellbeing People who use services are safe from harm People who work in health and social care services feel engaged and supported Resources are used effectively and efficiently

Core suite of integration indicators 10 indicators based on survey feedback Proportion of service users, carers and staff who rate the services highly and agree that the aims are being achieved 13 indicators derived from data collected primarily for other purposes Emergency bed days; readmissions; falls; delayed discharges; Care Inspectorate ratings; proportion of last 6 months of life spent at home or in community setting; discharges to care homes; expenditure
