What the Strategic Health Authority said worked well in Medway - Big Health Check 2010 Health Sub-Group Good ways of working are in place to set up a joint Kent and Medway way to buy specialist learning disability health services No-one in Medway currently living in campus accommodation Medway has good plans to increase the number of health checks that happen Medway is getting better information on the health of people with learning disabilities in Medway Medway Hospital has a Learning Disability acute liaison nurse

Still along way to go! Won’t be happy with health services for people with a learning disability in Medway until EVERYONE gets a good quality health service

Important to Remember! Medway’s targets were given before the government announced lots of the big changes it wanted to make for the health service For example G.P, consortia commissioning

Top Target 1 – Campus Closure and Resettlement Area to focus on: Develop local provision for people whose behaviours challenge services Establish a local network about people whose behaviour challenge services Evidence this has improved: No-one in Medway is in campus accommodation G.P. Consortia – network

Top target 2 – Reducing Health Inequalities Area to focus on: Medway needs a Joint Strategic Needs Assessment for Learning Disability Increase the number of health checks for people with a learning disability Evidence this has improved: Joint Strategic Needs Assessment - drafted Number of health checks carried out has increased by 10% to 61.4% this year Information (data) from GPs has got much better

Target 3 – Keeping Safe Area to focus on: ‘Join up’ work to make sure people are safe Carry on checking and improving health services for people with a learning disability Evidence this has improved: New Learning Disability G.P. register is helping ‘join-up’ information NHS Joint Commissioner Learning Disability in post

Target 4 – Transition, Community Team and Offender Health Area to focus on: 1.Transition 2.Community health team 3.Offender health 4.Medication and mental health

Target 4 – Transition Evidence this has improved: Transition Transition database set up Health and Medway Council 2011 Joint Review of children's disability services in Medway Getting a Life - Health pathway – this is being worked on Health is now represented at different levels of health planning

Target 4 – Community Team Evidence this has improved: Community Learning Disability Health Team 2 Away Days – Health and Social Care – independently facilitated by Rob Greig – Chief Executive National Development Team for Inclusion A draft service specification (what the team needs to do) has been written for the Community Learning Disability Health Team

Target 4 – Offender Health Evidence this has improved: Offender Health This area of work is happening as part of a ‘Kent and Medway Offender Health Plan’ Helen Gaylor – Medway Health Prison Partnership Board is a member of the Learning Disability Partnership Board

Target 4 – Mental Health Medication Evidence this has improved: Medication and Mental Health Mental Health Team have started to review the medication for people with mental health and learning disabilities

We have scored the targets for this year and these are shown on the next slide In the notes column we have explained why the scores have or have not changed We would like to work through the scores for this group to approve so that we can take the report to the LD Partnership Board When the LD Partnership Board has approved the scores we will send the report to the Strategic Health Authority Big Health Check Scores for Approval
