 Must validate syllabus on Bb  Must refer to CSD Majors site to access clinic handbook.  Use your handbook to for ANY questions regarding clinic policies / procedures / grades, etc.  You will be held to all clinic policies, procedures, rules and regulations noted in the clinic handbook.

Student Clinician Responsibilities  HIPAA – Confidentiality agreement  You have all signed the confidentiality agreement  Confidentiality with report writing  YOU MAY NOT WORK ON ANY REPORTS THAT HAVE NAMES OR IDENTIFIATION INFORMATION OUTSIDE OF THIS CLINIC.  Ask your supervisor how he/she wants you to save documents  Jump drives  Final printing of reports  All videos must be given to supervisor and any audio recordings must be deleted  CAA complaint process  If you have any problems or complaints about supervisors, clinical methods or any question about ethical / unethical behavior, refer to handbook

 You should be communicating with your supervisor regarding clinic schedules  You will be asked to call clients to set up therapy  ABSOLUTELY NO FILES / CLIENT INFORMATION LEAVE THIS BUILDING  PHONE NUMBERS OF CLIENTS SHOULD NOT BE LABELED WITH CONFIDENTIAL INFORMATION  EXAMPLE….  Once you have nailed down a time, the treatment room schedule must be completed outside of my office.  Check with your supervisor about blocked out rooms  ALWAYS write in pencil  All completed paper work from clients can be stored in your mailbox in the building until all is collected. Then, all paperwork should be given to GA in one stack  Get a WATCH – Cell phones off and put away  Don’t linger in rooms if sessions follow yours

Your Supervisor  Your assigned supervisor is your primary contact for all clinic needs  Ask how they want to be contacted  Who has contacted their supervisor already?  If you have not contacted your supervisor, YOU ARE LATE!!  Signs off on all clinic contact hours  For 500, 510:  Signs “Clinical Skills Checklist”  Completes the following grading forms  Session observation (optional)  Dx session observation (optional)  Midterm grade – modified CBS form / grading rubric  Final grade – modified CBS form / grading rubric  Best ways to contact supervisor – NOT through texting unless specifically stated.

Therapy Materials  Room 210 – Work Room  Room 203 –  GA, Dx Materials, Tx Materials  ALL MATERIALS MUST BE SIGNED OUT  Alert GA if sign out sheet is filled  The check out rule applies for Dx tests and materials from Rm 203 (48 hrs) unless you get special approval by clinic director  We are working on inventory  See supervisors for additional materials if needed.

Treatment / Therapy Rooms  You are responsible for keeping the rooms stocked with supplies and clean  We will know who is in the room at what times.  We have video running in clinic rooms at all times  Get in and out quickly if sessions follow yours  Disinfect  Remove your supplies after each session

 Buddy list – Use this FIRST  Entering and documenting  Monthly Attendance Record (MAR)  GRADS: Audiology hours must be logged within semester hours because of the site where they occurred.  Alert GA if you need your previous clinic hours logged in.  Its ALL on CSD Majors BB

Other Important Information  Staffings – arranged by supervisor – REQUIRED  Caseload – 2-3 clients to meet 40 hours  Clinic Service  Graduating Seniors  Must check out last week of classes with Mrs. Adams  Will receive a “Senior” memo  Emergency Closure  Clinic Applications – new form due February 18 th  Excel orientation - March

Planning  Review Client files  Keys / access / location of files  Room 210 will be student area  Files should not leave 2 nd floor of Campbell Hall  All files MUST be signed in / out, filed back correctly  Be early – everyday!  Professional dress (Wear what they wear) or CSD polos and pants  Be prepared with materials/resources appropriate for population served  Evidenced-Based Practice (EBP)  Review the clinic grading rubric so that you know what skills / behaviors are being graded and observed by your supervisor  Log hours DAILY  Use Common Sense!! Be Professional! Problem Solve!! Ask Questions!! You are being entrusted with someone’s well- being!!  Have fun, LEARN, and be responsible!!
