2 Contractor does not adhere to the PDO commuting policy by employing mapping of staff locations with development of associated Hubs. Possible exposure of commuting staff to transport risk and to sub-standard transport means. Recommendation: Project to confirm that Contractor’s activities regarding site area transportation of personnel provide sufficient protection as required by the PDO commuting policy HSE MS Audit Learnings

3 No assurance available for full compliance to legal requirements e.g. Environmental Permit conditions for Contractor STP. Recommendation: Project to specify responsibility and acquire assurance for monitoring and reporting of Environmental Legal Compliances HSE MS Audit Learnings

4 Hydro test pond/ evaporation ponds have perimeter fencing but there are no emergency escape ladders or Life Saving Equipment. Risk of drowning to personnel if accidentally entering the ponds. Access may be required for sampling or inspection purposes. Recommendation: Carry out a Risk Assessment of the hazards surrounding personnel access near the hydro-test / evaporation ponds, some of are which are deep (2+m), and the suitability of the controls and necessity for Life Saving Apparatus to be in place. 5 Helper employed on Radiography has personal Dosimeter but no records being maintained to evaluate cumulative dosages. Potential for undetected radiation exposure Recommendation: Project to confirm that the dosimeter readings of all radiation helper/workers (whether or not classified as a radiation worker) are recorded and monitored for cumulative dosages HSE MS Audit Learnings

6 The lifetime requirement of the temporary pipeline for the Aquifer Pump-Off (APO) facility is not identified. One of the options selected proposes the use of Shallow Water-well Disposal of potentially sour APO water. Some of the APO systems- eg: the temporary portion involving 3 APO wells- will be exposed to H2S of varying concentrations and may be vulnerable. However the project has demonstrated the ability to deploy HEMP in all other aspects and has indicated the intention of applying it to this issue. Recommendation: Project to confirm that the planned HEMP study will fully address the issue of sour water disposal in Shallow Wells and the permitting associated with this option HSE MS Audit Learnings
