Goal: Increase health promotion practices amongst seniors Objectives: To educate and encourage senior immunizations increasing vaccination rates among senior citizens by 30% To increase health awareness of our patients and their families by holding classes of 10 patients on a weekly basis. Goal: Maintain a holistic and interdisciplinary approach to patient care, by treating the whole person and not simply managing symptoms. The events coordinator will work along with the nurse educators and other team members to evaluate the educational needs of the elderly community. They will direct, coordinate and plan screenings and educational events for the organization. Secretary Events Coordinator Will oversee the functioning of the overall facility and ensure that the mission and values of the organization are being carried out efficiently. This individual is also responsible for maintaining the established ethical and legal standards for the workplace. Plays a role in attending meetings with other healthcare organizations to build and maintain partnerships with this organization. The nurse educator is responsible for developing an educational plan for the elderly on topics of disease process, nutrition, medications and physical exercise. These educators may work with a group of community members or may schedule one on one time to cover specific issues.
