1.Changing Disease Trend & Pattern  Incidence of infectious diseases  Emergence of new diseases & re- emergence of old diseases  Chronic & degenerative diseases  ageing population  changes in lifestyle and behaviour

2. Equity – the dimension of improving equity is viewed from 2 different perspectives Physical accessibility  Resources & facilities are unevenly distributed  Lack of private facilities in rural areas Cost access  Rural population spent less than urban population?  Prepayment mechanism prefered to out-of- pocket  high cost of care at private facilities
