30-Day Readmission Rate Hospitals/LTACH NUMERATOR = Among patients in the denominator, # readmitted within 30 days for any cause DEMOMINATOR = # of patients discharged alive during the time period, excluding transfers.

30-Day Readmission Rate LTCF NUMERATOR = Among patients in the denominator, – # discharged to a hospital within 30 days of the previous hospitalization – discharge for any cause. DEMOMINATOR = – # of patients discharged alive during the time period AND – also were discharged from a hospital within 30 days prior.

Required Collaborative Activities Example Month of APRIL Submit Monthly Data between May 21st & May 31st – Monthly CRE and Readmissions submission – Attend 2 Webinars. April 10 th and April 30 th – Train your staff using Train-the-Trainer presentation on CRE Basics – Begin planning PDSA cycles for required quality audit in June.
