MUSC Options No Longer Available MUSC Options, a health plan offered in Berkeley, Charleston, Colleton, and Dorchester counties, will not longer be available as of January 1, Subscribers enrolled in the plan should choose another plan. Those who do not make a choice will be assigned to the State Health Plan Standard Plan. 9

10 Changes you CAN’T make with health or dental insurance You CAN’T add or remove your spouse or dependent children unless you have a qualifying event to do so and it is within 31 days

Dental Insurance This year employees are not able to make changes in the dental plan Dental changes will be allowable during Open Enrollment, 2010 Dental Plus Premiums are increasing 11

12 Changes in Optional Life Insurance Without medical evidence form: Employees may enroll and/or increase their optional life insurance for up to $50,000 in $10,000 increments Employees without optional life insurance may enroll for up to $50,000 in $10,000 increments Employees can enroll children for $15,000, $1.24 per month for any number of children Employees can always decrease or cancel Optional Life or Dependent Life coverage

Changes in Optional Life Insurance With medical evidence Employees may enroll their spouses in $10,000 increments for up to 50% of their Employees Optional Life coverage or $100,000, whichever is less. Employees may enroll for above $50,000 with medical evidence 13

14 MoneyPlu$ Programs To participate in MoneyPlu$ accounts in 2009 You must enroll or re-enroll You can do this online You must meet eligibility requirements of having been employed by January 1 st 2008.

15 HSA (Health Savings Account) You must be covered by the State Health Savings Plan or another high deductible health plan You cannot be covered by any other type of health plan, including Medicare (Part A or B) and you cannot be claimed as a dependent on another person’s tax return If you were enrolled in an HSA in 2008, you do not need to re-enroll in 2009 if you remain eligible but you may change the amount you contribute monthly

16 Dependent Care Account You must enroll or re-enroll in this if you wish to participate in 2009 You may deduct up to $2500 if on taxes you file married, filing separately You may deduct up to $5000 for single, head of household or married, filing jointly You may not be refunded for $$ you have not yet put in

17 Medical Spending Account You must enroll or re-enroll in this if you wish to participate in 2009 You may receive reimbursement for eligible medical expenses incurred by you or your family members up to $5000 maximum annually You may be refunded for $$ you have not yet put it in. For example, if you enrolled for $3000 and you have surgery costing $3000 on 01/01/2009 you are eligible to claim and receive the full amount.

Medical Spending Account MSA subscribers may sign up for an EZ Reimburse MasterCard. Check box on enrollment form Not available for limited-use MSA Advantages include instant reimbursement for health care expenses, instant approval of some medical, prescription, vision and dental expenses, easy access to your account funds 18

Increased Health Plan Premiums For tobacco users, premiums will increase in 2010 Free and Clear ® Quit for Life State Health Plan subscribers and covered dependents ages 18 and older may participate at no charge A research-based program that has been successful in helping people stop tobacco use. For more info call Quit-4-Life ( )

20 Health Management Program Blue Cross identifies participants for the program from healthcare information received from medical, pharmacy and lab claims. If you are identified as someone who could benefit from the program, you are automatically sent a welcome letter giving you more information. Examples the program is designed to assist with… diabetes or asthma

21 Mothers-To-Be You must call Medi-Call within the first trimester to enroll in the Maternity Management Program Once you are enrolled in the maternity management program, you can correspond online with your case manager and receive relevant articles from recognized medical resources.

22 Long Term Care Prudential is our New Long Term Care Provider If an employee was already covered under AETNA they will automatically receive a package sometime in September telling them what they need to do to change to Prudential If an employee was not covered under AETNA and wants to enroll, Prudential is holding a ONE TIME Open Enrollment from February 16-March 6,2009 to enroll employees with no medical evidence forms. Premiums will no longer be payroll deducted so employees will be directly billed for the service

Annual Enrollment October 1 st through October 31 st, 2008 Employees may make annual enrollment changes on MyBenefits, 24 hours per day, 7 days per week and select “My Benefits” Employees may also come to 1 MP, Suite 340 for assistance from HR to make any allowable changes 23
