Part 1: Deciding if you have something to disclose DO NOT REPORT: Research funding paid through UM / ORSP. Government work (e.g., NIH study section/grant review, FDA device review). Payment/travel/honoraria for speaking at a non-profit, US academic entity. Professional Society service/work where you are NOT: in a leadership role; paid; travel is paid. If any of these apply you must report. 3

Part 4 Making a New Disclosure & Updating Last Year’s Disclosures 18
