Clark County Agencies In Clark County, there are 3 different applications One for Clark County Health One for Clark County Income One for Clark County Education It is your responsibility to choose into which impact area your program falls. Some agencies have several programs and may have to use all links. Some agencies may have more than one program in the same impact area. * In Champaign and Madison Counties, there is only one application

In The Application All fields are required and there are some text limited fields We require one attachment of the budget. Our budget sheet is also on our website. You can apply for up to 3 programs within the same application. Each program is on a separate page If you are applying for only one program skip over pages 4-6 and submit application, etc. You may only use your account once per application link.
