Patient Transferring within and between healthcare setting

objective By the end of this lecture you will be able to : By the end of this lecture you will be able to : Explain the ideal process of patient transferring.

When you transfer a patient to another unit or to another facility, you are responsible for carrying out certain procedures will make the transition smooth and nonstressful as possible for the patient and receiving staff. this include: 1- obtaining the order for transfer 2- notifying appropriate persons 3- interacting with the patient, including reinforcing any health teaching.

4- caring for the patient's personal property. 5- completing and duplicating records,assessment data, and care plan 6- arranging transportation. 7- giving report.

obtaining the order for transfer Physician usually writes the order transferring a patient within the hospital or to another facility. If there is some nursing reason for transfer, the transfer is made and the physician writes the order later.

notifying appropriate persons You are responsible to notify all persons involved of the transfer. These include the nursing supervisor, other hospital departments ( eg, diatory, pharmacy or physical therapy ), the patient family, and the unit or facility that is receiving the patient. interacting with the patient Any patient has the right to know of the transfer plans. You can clarify the transfer and answer any question the patient or family may have regarding the new environment.

caring for the patient's personal property. Make sure that all personal property is checked and accompanies the patient being transferred when appropriate.

completing and duplicating records,assessment data, and care plan If the patient is being transferred within the facility, gather all assessment forms and care plans, bedside equipment, and plastic identification card.

arranging transportation When critical care needed, the nurse may make arrangements, particularly when the patient needs special equipment, such as a oxygen or maintenance of intravenous a therapy, or specialized personal, such as a respiratory therapist or critical care nurse.if pt is going to another unit, you may decide whether the pt can be transferred by wheelchair or stretcher.

giving report The nurse in original area gives a verbal report to the nurse in the new area. The nurse in original area gives a verbal report to the nurse in the new area.

Transfer to a long –term care facility When the pt is being transferred to another hospital or to another facility (long –term care facility e,g retirement center, residential institution, nursing home …),

General consideration - The original chart, remains at the hospital. - All of patient's belonging are carefully packed and sent to the facility with patient.

Con.. - Prescription and appointment cards for return visits to the physician is sent to the facility.. - Detailed assessment and care plan is sent to the facility. - The nurse at hospital often provides a verbal report to the nurse at long – term facility.
