A health insurance plan’s actuarial value indicates the average share of medical spending that is paid by the plan, as opposed to being paid out of pocket by the consumer The calculation takes into account various plan features: –Cost-sharing elements, such as deductibles, coinsurance, copayments, and out-of-pocket limits, and –The range of services covered by the plan For example, an actuarial value of 70% means that, on average, the plan pays for 70% of medical spending and consumers pay the remaining 30% out of pocket

Copyright © 2011 by the American Academy of Actuaries 4 Actuarial value under the ACA Under the Affordable Care Act (ACA), actuarial value will be used to categorize plans by benefit tiers –Platinum = 90% actuarial value –Gold = 80% actuarial value –Silver = 70% actuarial value –Bronze = 60% actuarial value There is no uniform method of determining an actuarial value The appropriate method will vary depending on the goal of the calculation

Copyright © 2011 by the American Academy of Actuaries 5 Identifying potential primary goals Option 1: Plans with identical coverage and cost-sharing provisions should have the same actuarial values –Actuarial value differences between plans would reflect only plan design differences Option 2: A plan’s actuarial value should correspond more directly with the expected share of spending paid by the plan –Provider discounts and utilization patterns of each plan would be incorporated into the actuarial value calculation

Copyright © 2011 by the American Academy of Actuaries 6 How provider payment rates and utilization patterns could affect actuarial values Actuarial values are higher when health spending is higher –Any fixed dollar deductibles and out-of-pocket limits are more likely to be exceeded, above which the plan pays a greater share of spending Plans with lower total spending—through lower provider payments or use of less costly services (e.g., generic drugs)—would pay a lower share of total spending, all else equal –Under Option 1, no impact on actuarial value –Under Option 2, lower actuarial value

Copyright © 2011 by the American Academy of Actuaries 7 Neither approach provides guidance on out- of-pocket dollar amounts Option 1 ignores provider payment rates and utilization management differences between plans –Total allowed costs, and therefore the actual cost-sharing requirements, could be under- or over-stated Even under Option 2, the share of costs paid by the plan won’t necessarily be correlated with the out-of-pocket dollar amounts –For instance, plans with lower provider payment rates could have lower actuarial values and lower out-of-pocket dollar costs

Copyright © 2011 by the American Academy of Actuaries 9 Data options Option 1: Use common standardized dataset –Would facilitate goal of achieving similar actuarial values for plans with similar plan designs Standard assumptions may be needed regarding how utilization changes under differing cost-sharing requirements –Considerable effort required to develop appropriate dataset Option 2: Allow plans to use their own data, normalized using risk scores –Would facilitate goal of better reflecting the expected share of spending paid by the plan –Could be more administratively feasible

Copyright © 2011 by the American Academy of Actuaries 10 Other considerations Defining medical spending –Medical spending is defined according to the essential benefits package –The more broadly the essential benefits package requirements are defined, the less clear it becomes regarding which spending is included in the actuarial value calculation Geographic variations –Incorporating regional variations in health spending would result in actuarial values that better reflect local markets, But cross-area comparisons of plan designs would be more difficult

Copyright © 2011 by the American Academy of Actuaries 11 Final thoughts Method for calculating actuarial value needs to strike a balance between helping distinguish plans by coverage tiers and administrative simplicity Because actuarial value calculations are done on an average basis for a given population, different plans may be more or less valuable to any particular individual, even among plans in the same benefit tier
