Florida Department of Health Asthma Program & The Florida Asthma Coalition 4 Asthma-Friendly Schools Recognition Home Visiting Demonstration Project Facilitating Asthma Care Teams Establishing Learning and Action Networks

Purpose of the LAN Better care Smarter spending Healthier People To produce statewide improvement in delivering smoking cessation opportunities and comprehensive asthma management. 5

Million Hearts Initiative 6 To prevent 1 million heart attacks and strokes by 2017 using a LAN approach. 1. Empowering Americans to make healthy choices 2. Improving care for people who do not need treatment by encouraging a targeted focus on the “ABCS” Image Source: Million Hearts,

Participating in the LAN  Open invitation to participate for MMA standard and specialty plans  The LAN contractor will bring together the group using a variety of contexts  4-6 meetings via webinar or online forum  Topics will be driven by YOUR interests  Benefits include:  Financial support  Performance and quality improvement  Public health and health care system linkages  Collaborative discussion of decision support tools and electronic health records for care coordination 7
