Open Enrollment Process Review the 2014 Benefit Decision Guide Contact JA Counter for one on one assistance Complete a Medica enrollment/change form to change your current medical plan election Current election will remain in effect if no form received If participating in the medical insurance, complete a HSA payroll contribution form Complete a 2014 Flex election form Contract the District office if you wish to change any other plan elections
Open Enrollment Ends December 1, 2014 All forms must be returned to Elaine before the end of Open Enrollment
Moving Forward Contact Human Resources within 31 days if you experience one of the following Family Status Changes during the year Marriage/Divorce Birth/Death Loss of other coverage Change in employment for you, your spouse or dependent Changes outside of open enrollment will only be allowed if they are consistent with a Qualifying Family Status change & the District Office is notified within 31 days of the event
