“Modelling the Supports” Outcomes: Person Centred Plans clearly stem from an individual’s desire to develop a positive change in life. To identify whether meaningful progress has been made in utilising Person Centred Planning a number of different measurement strategies could be employed. The challenge for all is to clearly link the actions of the service (what the service does or ‘outputs’) to what changes as a result (outcomes) Measurement in these terms provides a view of change in the short term, medium to long term and also focuses the dialogue on the broader impacts of any changes made. Case Studies of Best Practice
