About the Agency 13 Employees Created in 2005 Serves about 2,500 people each year Perry & Harlan Counties Four Sites Serves homeless people in the region with primary care, mental health & substance abuse services, preventive care, disease management and enabling services.

Rotary Free Clinic Without Walls History: A group of individuals concerned about the difficulties individuals face accessing health care had a conversation over a luncheon. From this luncheon emerged Rotary Free Clinic Rotary Free Clinic Board was incorporated in 1999 and it is primarily sponsored by Rotary Civic Club

Rotary Free Clinic Without Walls How it works: The Rotary Free Clinic Board of Directors and KMHA staff actively recruit providers It provides an opportunity for physicians to see a designated number of physicians each month. Providers are enrolled by completing enrollment packet Patients can be screened and referred from KMHA or Little Flower Clinic Patients are given a voucher (pink slip) Number of referrals, visits and visit outcomes are tracked through logs Additional referrals are made if necessary and medication are obtained Reports are provided to referring providers with follow-up instructions
