Conference- Pathfinder update Oct 12 Stuart Hall and Layla Bridger Improving Services for Disabled Children

The old/current customer experience

Improving Services for Disabled Children Parents and carers feel they are facing a system where they have to battle for the support they need, where they are passed from pillar to post, and where bureaucracy and frustration face them at every step.

Improving Services for Disabled Children

So what we did…. Families experiences within particular areas were often really good, but when support was required across more than one team, generally the family experience was not so good. Work on the Disabled Children and Adults (DCA) review started 2011 to improve this Engaged with 150 staff across social care (children and adults), local authority education services and health services Engaged with 80 customers face to face and received 240 returned electronic surveys Wiltshire applied to become a pathfinder in August Multiagency bid. Wiltshire one of 20 successful applications Asked to ‘pathfind’ on single assessment process and personal budgets The work of the DCA review and pathfinder has merged, now the DCA Pathfinder

Improving Services for Disabled Children DCA pathfinder- our achievements Co-production with children and young people and parent carers. Co-production with design team Agreement to design integrated service that brings together elements of children and adult social care, education and some targeted services to support children and young people with disabilities and Special Educational Needs aged 0 to 25 (stability) Development of new paperwork to facilitate the single assessment process and plan Development of a Special Educational Needs and Disability (SEND) Lead Worker

Improving Services for Disabled Children Establishment of “proof of concept team” Initial feedback from families very positive Training underway –Person centred –Key working DCA pathfinder- our achievements cont:- Hello my son is called Dave. He finds any change really difficult and is going to be 18 next year. I was really pleased when we were asked to be involved in this project. At our meeting there was someone from education and a social worker., including one from adult care This has never happened before. We are planning what happens now and for Dave’s future. I liked the idea that all the people were in the same room and I didn’t have to say things to different groups of people.

Improving Services for Disabled Children Challenge Focus on age 0-25 Not 0-18 and 18 + Across social care, education and health Starting to blur the boundaries Practitioners Children and young peopleParent/carers

Improving Services for Disabled Children Next steps WPCC working hand in hand with Local Authority WPCC Pathfinder Event Families will test, develop the structure and model which will in turn … Inform the Local Offer Training of Staff
