Accident Reporting

Ouch! I’ve Had A Job Related Accident & Injury What happens now? Immediately report it to your Supervisor or Manager – By Law, late notice or failure to notify may bar a claim for Workers’ Compensation Benefits! and get medical treatment (if required). Complete the employee portion of the ‘First Report of Injury’ form. Cooperate with any investigation by Management. Return to work (if possible).

First Report of Injury Form

Used to provide personal information and describe the specifics of the accident & injury Complete the form even if the injury is minor and only requires first aid You may be contacted by the Corporate Workers’ Compensation Administrator to verify the information provided Why?

Investigation of Accident & Injury Initiated promptly by management Discuss with any and all witnesses Document findings of what happened and how to prevent future incidents Review policy and procedure to determine if there was an infraction and any consequence Perform remedial training in safety policy and procedure when and where needed

Returning to Work Immediate if no medical treatment required or only minor in-house first aid performed If outside medical treatment is required: You will be post accident drug tested if injury is self-inflicted If unable to work and medical leave is required, bring a doctor’s written statement with diagnoses and estimated return to work to your supervisor
