Eligibility Requirements (Pg. 2) Wellness Program Enrollment Form (Deadline November 2013) -New Hire (Hired before May 1 st 2014 must complete “All” requirements, however those hired after May 1 st 2014 will be expected to do as much as they can if signed up for the program). -Health Assessment (Must be completed April 1 st – May 1 st ) -Health Assessments will be accepted NO LATER than May 1st -On-line or Paper form -On-line instructions - -Click on “SEWS” main tab -Click on “Health Assessment” -Password is “wiredup” -Blood Work (Deadline June 1 st ) -Done on site May 1 st – June 1 st, (Any blood work completed and submitted to One Stop Wellness January 1 st - June 1 st, 2014 will be accepted for the Wellness Program). -Blood work can be submitted by or fax to

“Wired Up” Quarterly Agenda 1 st Quarter (Pg. 3 & 4) -Launching of 1 st 10 Week On-going Health Management Program “Walk the Talk” -Jan 22nd – Apr 2nd - “Walk the Talk” – This 10 week program will track your activity. Don’t worry… we’ll have information to help you be successful. Have you thought about looking at your activity in minutes per week rather than the specific activity? We’ll focus on putting the “time” in. There will be a weight loss component to this for those wanting to drop the pounds. -”Stress Busters” Part 1 Launching this 6 part On-going Health Management Program “Stress Busters”. This is an all year program -”Stress Busters” Part 2 -January, February, and March Newsletter -Webinars (Completed on-line at your leisure at -Lunch-n-learns (Conducted by each individual site in a group setting at your locations discretion)

2 nd Quarter (Pg. 3 & 4) - Launching 2 nd 8 Week On-going Health Management Program “Targeted… Glucose and Cholesterol” -Apr 2 nd – June 4th -This program gives you the opportunity to “Target” two of the numbers on your blood work for cold hard cash. If your glucose and cholesterol is already in range, simply maintain this number. If it is out of range, simply lower it by 7% to receive credit. You will be rewarded for each or both of these numbers if you meet the criteria of the program. -Stress Busters Part 3 -Stress Busters Part 4 -Health Assessments (April 1 st – May 1 st 2014) -Blood Work (May 1 st – June 1 st 2014). -April, May, and June Newsletter -Webinars (Completed on-line at your leisure at -Lunch-n-learns (Conducted by each individual site in a group setting at your locations discretion)

4 th Quarter (Pg. 3 & 4) -Deadline for Completing 2014 Action Items (November 1 st, 2014) -Reenrollment into the 2015 Wellness Program (November 2014) -You must have completed your 2014 Eligibility Requirements to receive a benefits discount in Reporting (aggregate data regionally for Blood Work and Health Assessments) -October/November/December Newsletter - Webinars (Completed on-line at your leisure at -Lunch-n-learns (Conducted by each individual site in a group setting at your locations discretion) -Planning 2015 programming
