The work of the London Borough of Ealing Transition Board Report to the Learning Disabilities Partnership Board on 23 November 2011

Who comes to the meetings Service users, parent carer reps, Mencap, Connexions, Children with Disabilities Team, SEN Team, CTPLD (adults team), school reps, college reps, health reps, Ealing Contact-A-Family, Support for Living.

The Transition Board continues to work on these 3 themes How we all work together to make transition better for young people. How we involve young people and their families so that they are in control of their own plans. Real Opportunities for Young People.

What is going well - Information Day & Evening information sessions for parents as well as a monthly transition drop in at the Carers Centre so parents know what to expect. The EalingHelp website has improved information for parents & is a portal to parents to have online discussions. Young people have been involved in the first ClickStart training for professionals. There have been forums for parents at Southall Contact A Family & a parent session in partnership with ContinYou.

What is going well – Paid Staff The virtual transition team continues to meet at Carmelita House. There has been a review & a new agenda is being set. ESCAN has promoted Health Action Plans being piloted in special schools. Now have more people with person centred plans in mainstream & special schools. Membership on the Children with Disabilities Partnership Board & on the Strategy Group.

What is going well – Young people & carers Parent Information Pack is available at various forums e.g. Drop-ins. Young people at the New Start Group continue to feed ideas to the Transition Board. Parent/ carers at Transition Board linking well with the Parent Forum.

What is going well – Opportunities New transition worker through the Synergy Project, working with young people Now a wider range of options for young people to purchase, using their individual budgets. Young people are accessing employment opportunities via Cultivate London and hold regular market stalls. The housing allocation quota for people with learning disabilities is now 20. x20

Young people have roles in training, buddying & mentoring around the borough. Young people are planning the annual ‘What Next’ event, taking place on 8 December. Opportunities also include the Art Project, the band ‘I Love Thunder’ & deejaying around Ealing. Young people took part in the annual Big Health Check Day & are getting paid to be GP assessors. The revamped Westside Youth Centre is now fully accessible for young people with disabilities. What is going well – Opportunities

What are the challenges? People continue to need help to understand individual budgets & how they can be used. We need new post 19 local learning opportunities for people with high support needs. Making sure there is a wider range of short break services for young people.

What are the challenges? To clarify the process for transition from children to adult services. Difficulties in getting an assessment for young adults in a timely manner and parents to be informed of the outcome in a timely manner. Continue developing Health Action Plans for those moving out of children’s services. ?

What are the challenges? GPs need to become central to helping young people with disabilities stay healthy. Finding accessible, affordable housing locally. Finding new work opportunities when youth unemployment is high. Starting social firms when money is tight. Ealing
