Part 2 - Visit the UF Desk  Individualized MHI Results: Find out what percentile your practice is in!  Finalize Details of Site Visits: Timing of visit Space to accommodate 5 people Look at Staff Survey packs Look at Interview questions  Quality Checks: Practices: 1, 2, 3, 5, 6, 7, 8, 9, 10, 12, 13, 15, 18, 19, 20

Next Steps – Part 1 (QI project)  Complete Learning Session Evaluation Form  Complete Travel Expense Form and return to Lindsay Wilson to arrive no later than September 30, 2011  Claim CME Credit (Tab 3) Live Learning Session: 21 days after the session Performance Improvement: After 1 data cycle is complete (staff will you)  Prepare practice  Begin monthly data collection: Data cycle opens October 15 and closes October 30 Submit first 10 charts of Medicaid, Medicaid health plan, or KidCare via EQIPP Submit Monthly Progress Report via Survey Monkey Links to data collection requirements will be available on the Project Workspace Web site and will be ed to teams each month  Participate on Monthly Conference Calls (Practice members only)
