Introduction Introduce who you are (region/city/district, who your team members are and titles, where your program was at the beginning of this year, and any other information that is important to know who you are and what is your focus). It is also good to include baseline data here.

Objectives Discuss what were your goals and objectives for this year

Methods, Activities, and Procedures This section should include: – Design of your program – Target population (who participated) – Instruments used (I-13 checklist, pretest-posttest, etc.) – Sampling procedures (evaluations, questionnaire, survey, interviews, focus groups, etc.) – Data collection procedures – Data analyses procedures – Validity and reliability – Limitations

Results Description of findings from data analyses. The discussion of findings does not go in this section just the facts. This is where your charts and graphs go.

Results

Conclusions Interpret the results and findings of your data analysis. Give conclusions about effectiveness. Tell what worked, what didn’t work and why.

Recommendations and Next Steps Discuss your recommendations based on the data and provide the next steps.
