Benefits of Diagnosing UTI n Treat current illness –Make patient feel better –Prevent worsening (development of bacteremia/sepsis) n Prevent future sequelae –Identify patients who may benefit from imaging to detect underlying abnormalities –Identify patients who may benefit from prophylactic antibiotics

What happens if you miss a UTI? n PROS Febrile infant study: 807 febrile infants <3 months old –no urine test first visit –not treated with antibiotics –61 UTIs expected –Only two later diagnosed; both did fine n Conclusion: even in this high-risk age group most UTIs resolve spontaneously *Newman TB et al. What happens if you miss a UTI? n Prospective cohort study of 15,781 febrile illnesses in children < 5 years old n 1140 cases of serious bacterial infection (UTI in 543, Pneumonia in 533, bacteremia in 64) n 363/1140 (32%) not initially treated –230 subsequently treated –Only 8 (1.4%) “unwell” at follow-up average of 10.2 days later, none febrile –Only death was in a child with terminal illness *Craig J et al. BMJ 2010;340:c1594

Patients most likely to benefit from early UTI diagnosis n Younger patients (<6 months and especially < 3 months) n Sicker patients (higher fever, more irritable) n Patients who have been sick longer n Patients with evidence of underlying disease, e.g., recurrent fevers of uncertain origin, poor growth, poor urine flow, antenatal renal abnormality n Patients for whom follow-up is difficult

Recommendation #1: Obtain a urine sample when level of concern about UTI is high, e.g.: n Child appears acutely ill without source n Illness lasting > 2-3 days or recurrent unexplained fevers n Age < 3 mos n Underlying disease or difficult follow-up n Significant risk of UTI (10%) –Girls, no source T>39 –Boys 3-12 months if uncircumcised n Include parents in the decision if culturally appropriate

Decision #2: Imaging after UTI n What are you looking for? n How much cost and risk is it worth to find it?

After a first uncomplicated febrile UTI in a 15 month old girl with no other indications, would you recommend a VCUG A. Always B. Almost always C. Usually D. Sometimes E. Rarely F. Never

Abnormalities on Urinary Tract Imaging n Obstruction -- treatable n Vesicoureteral reflux --- questionable benefit of treatment n Duplications of collecting system – no management change n Scarring- little or no management change

Renal Ultrasound n Safe, noninvasive, no radiation n Can identify significant obstructive lesions n May miss duplications, reflux n Low yield (obstruction or change in management 0/309*), especially with normal 3 rd trimester ultrasound n Charges (UCSF, 2010) $1214 *Hoberman et al., NEJM 2003

99Tc-DMSA Scan n Measures (relative) function n Abnormal acutely in pyleonephritis n Relatively noninvasive n “Scarring” in ~10%, but rarely changes management n Charges (2010, UCSF) $1813 without SPECT, $4856 with SPECT

VCUG or RNC n Involves urethral catheterization fo fill bladder –Uncomfortable, frightening, risk of introducing infection n Only way reliably to identify VU reflux n RNC lower radiation dose, but lower resolution and can’t see urethra n Used to be widely recommended n Charges (UCSF 2008) $1306 –($280 Pharmacy charge for contrast!)

Rationale for Treatment of VU Reflux n Reflux + Infection  Scarring n Scarring  HTN, Preeclampsia, Chronic Kidney Disease, ESRD n Prevent sequelae by fixing reflux (surgery/Deflux®) or preventing infection (prophylactic antibiotics) n Inconsistent evidence to support this rationale
