Introduction This resource is intended to assist health care providers working with stroke patients and reflects the 4 th edition of the Canadian Best Practice Recommendations for Stroke Care (Update March 2013) 1. Recent reports have shown that there is inadequate screening and monitoring of patients for vascular cognitive functioning issues 2. It is hoped that the content in this resource will assist health care providers in Taking Action, a theme of the 4 th edition of the Canadian Best Practice Recommendations for Stroke Care, recognizing the importance of screening in practice for vascular cognitive impairment 1.

Introduction This resource has been organized into 7 chapters in addition to supplementary modules so that individuals can readily navigate to the chapter or module of interest.
