ISCOE - Goals Identification of promising online STI/HIV prevention interventions (at client level) and resources (at provider level) Adaptation and evaluation of these interventions for use in program/clinic (rather than research) settings Development of online and offline tools to assist with adoption of these interventions at the program level

ISCOE - Setting Center is located at Denver Public Health Department with support from CDC –Strong background in in STD/HIV-related Internet research –Close proximity of the Denver Metro Health Clinic (DMHC): the largest STD clinic in the region allows for evaluations at the level of, relatively easily accessible at-risk populations

ISCOE Activities for Online Interventions at Client Level Data collection for assessment and evaluation: –DMHC “roving” survey Topics: Internet access & dating, Cell phones & text messaging, use of Internet for dating, STD/HIV information, partner notification, online testing –Linked to electronic medical record system Demographics Risk factors Diagnosis and treatment data –Client focus groups

Project: Text Messaging “Can You Hear Me Now?” Purpose –To determine if health messages delivered using mobile phone text messages would be acceptable and feasible to improve return rates among STD clinic patients: Within 2 weeks after a positive STD diagnosis For re-screening 3 months after treatment for gonorrhea or chlamydia

Cell Phone & Text Messaging Use Among STD Clinic Patients Clinic Survey Results Do you use a cell phone?84.7% Do you use text messaging on your cell phone? 90.2% Is it OK to contact you via text message?93.5% Presented at ISSTDR, 2007

Focus groups findings The message must look professional No emoticons, no abbreviations Keep it discrete No clinic identifiers “Please call Denver Health for an important health message” Enthusiastic about text messaging Presented at ISSTDR, 2007

Text Messaging Project Status Project went live in October –Patients give specific consent –Reports of qualifying patients automatically generated on daily basis –Text messages sent to patients with gonorrhea and/or chlamydia who did not call for results > 1 week –Text messages to encourage re-screening will start in February

Project: Providing STD Test Results Online San Francisco STD Clinic: proof of concept: –Substantially improved number of patients receiving test results –Substantially decreased amount of staff time to give in-person or telephone results –However, SF clinic has larger proportion of MSM than other STD clinics  generalizability?

Providing STD Test Results Online Project Description Objective: –To develop a “generic” results engine that could work across different platforms If proven effective could be disseminated across different clinics with (hopefully) minimal adaptations –To evaluate the use in the Denver STD clinic environment How many patients request online access by submitting password information? How many patients access results online? Of these, how many return to the clinic for treatment (if needed)? What, if any, impact on clinic staff?

Providing STD Results Online Project Status Results engine is built Go-live date: February Another project within Denver Public Health interested in using the engine to provide HIV viral load and CD4 count results for an online intervention with HIV+ individuals  will test usefulness of the engine in a different environment
