Purpose These funds are being invested in health centers that did not receive O/E funding in FY 2013 and beyond to expand O/E assistance activities and facilitate enrollment of eligible health center patients and service area residents into affordable health insurance coverage through: –Health Insurance Marketplaces; –Medicaid; and –The Children’s Health Insurance Program. 3

Summary of Funding HRSA will award approximately $8 million under the FY 2015 O/E assistance funding opportunity through formula-based supplemental awards to Health Center Program grantees that did not receive FY 2013 O/E Assistance Supplemental Funding. Requests for Federal support must be reasonable and appropriate based on the scope and breadth of the O/E assistance activities being proposed and the number of patients and residents of the service area projected to be reached. 4

Funding Opportunity Requirements Health centers receiving O/E supplemental funding must: 1.Increase the health center’s current outreach and enrollment capacity. A minimum of at least 1.0 full- time equivalent (FTE) must be added to the health center’s current O/E assistance capacity. 2.Ensure all health center O/E assisters (i.e., current and newly supported) comply with and successfully complete all required and applicable federal and/or state assister training and comply with all associated regulations and requirements, as are required for personnel carrying out consumer assistance functions. 5

Funding Opportunity Requirements 3.Demonstrate the capacity to conduct “in reach” with currently uninsured health center patients and “outreach” to non-health center patients in their approved service area. 4.Describe collaboration with other health centers, PCAs and providers in their service area to ensure that O/E assistance activities are coordinated with other local, regional, and/or state-wide O/E assistance efforts and training requirements. 5.Comply with all applicable Health Center Program requirements. 6

Funding Methodology Funding allocations were based on the following: –A base amount of $54,000; –An additional $5,000 for one-time expenditures; and –An additional amount based on the grantees’ proportion of uninsured patients, as reported in 2013 UDS. Depending on the number of approvable applications, HRSA may make adjustments in award amounts consistent with funds available for this supplemental opportunity. Cost sharing or matching is not a requirement for this funding. 7

Eligible Applicants Organizations receiving Health Center Program operational funds (section 330 of the PHS Act, as amended) that did not receive O/E funding in FY 2013 are eligible to apply for FY 2015 O/E assistance supplemental funding. Health centers that have been determined and notified by HRSA as materially noncompliant with Health Center Program requirements are not eligible. Health centers that received initial Health Center Program grant funding through a NAP opportunity in FY 2013 or FY 2014 (e.g., new start NAPs) that have not verified at least one site in scope is operational and providing services as of the date of this funding opportunity release are not eligible. 8

Allowable Uses of O/E Funds Personnel, including at least one FTE O/E assister who will be trained as a certified application counselor or equivalent to facilitate enrollment in new affordable insurance options available through the Marketplace, Medicaid and CHIP on behalf of the health center. Fringe benefits for O/E assisters Moveable equipment and supplies to support outreach and enrollment assistance individually valued at less than $5,000 Leasing of temporary space for the sole purpose of O/E assistance activities (e.g., leasing space in a community center) Paid media, mailers, and other broad-based strategies to promote the availability of health center O/E assistance to non-health center patients Training in support of the grant Local travel in support of the grant 9

Unallowable Uses of O/E Supplemental Funds Fringe benefits other than those to support O/E assisters; Incentives (e.g., gift cards, food, free services); Moveable equipment and supplies individually valued at $5,000 or greater; Vehicles; Primary health care services; Construction costs (including minor alterations and renovation and fixed/installed equipment); and Facility or land purchases. 10

O/E Supplemental Funding Application and Due Date The FY 2015 O/E supplemental funding application requires completion and submission of the following within HRSA’s Electronic Handbook (EHB) by October 6, 2014 at 5:00 PM ET. 1.Face Page and Standard Forms 2.Project Narrative and Projections 3.Work Plan 4.Budget 5.Budget Narrative Justification 11

EHB Application Each eligible grantee received an detailing how to access the application in EHB on Wednesday September 24, notifications were sent to the organization’s Authorizing Office (AO), Business Office (BO), and Project Director (PD) 12

Project Narrative Project narrative must include a brief description of: 1.How the health center will use O/E supplemental funding and leverage current resources to meet the eligibility assistance and enrollment needs in the health center’s approved service area; and 2.How the health center will coordinate O/E assistance efforts with other health centers (grantees and look-alikes), PCAs, and with other state, local, and/or regional efforts. 13

Projections Required projections include the following: # New FTEs: Projected number of additional funded health center assister FTEs; # Trained: Projected number of health center assisters who will successfully complete all required and applicable federal and/or state training to assist individuals with enrollment through federal, state-based or state partnership marketplaces. # Assists Provided: Projected number of assists provided by trained health center assisters; and # Enrolled: Projected number of individuals estimated to enroll through the Marketplace, Medicaid, and CHIP with the assistance of trained health center assisters. 14

Work Plan An optional work plan template is available on the O/E TA website. Activities for the work plan may include: –Develop outreach and enrollment assistance coordination/collaboration plans; –Recruit and hire new outreach and enrollment assistance workers (expected within 30 days of award); –Collaborate with state and/or regional PCA to identify state-specific training and other requirements necessary to support O/E activities; –Successfully complete all required and applicable federal and/or state consumer assistance training; –Perform outreach and enrollment assistance activities; –Evaluate progress and revise strategies as appropriate; and –Submit quarterly progress reports beginning in January

EHB Submission and Validation Submission of all application forms will be completed electronically in HRSA’s EHB by 5:00 PM ET on October 6, FY 2015 O/E supplemental application can only be submitted by the health center’s Authorizing Official (AO). Applicants will receive an “Application successfully transmitted to HRSA” message in EHB. For help with electronic submission in EHB, call the BPHC Helpline at

FY 2015 O/E Supplemental Funding HRSA will conduct reviews for eligibility, completeness, and allowable costs. HRSA anticipates awarding FY 2015 O/E supplemental funding in October Recipients of FY 2015 O/E supplemental funding must comply with applicable requirements of section 330. Health centers must submit quarterly progress reports (QPR) to HRSA and document progress including: –Number of O/E assisters trained –Number of assists provided –Number of applications submitted –Number of individuals estimated to have enrolled –Issues/barriers and successes/lessons learned A sample QPR form is available at: 18
