Youth Transition scenario 17 year old medically complex youth with chronic health issues Has Medi-cal insurance Dependent on others for care Frequent hospital admissions Current care includes his primary physician and 5 specialty clinicians Attends school

Ideal planning age Because of the medical complexities of this child an ideal age would be between years old

Transition concerns Medical care Equipment needs Medical supplies Therapeutic services School program

Who should be involved in this process? Parents Primary physician Specialty physicians School district Regional Center case manager CCS therapist and nurse case manager
