HORIZON 101 BY DR. JIM KERR AND DR. WARREN TOEWS

CO-MANAGEMENT We have a unique patient care system that involves the optometrist, and the participating ophthalmologists. We provide the safest technology at our center and the highest quality of care through this care system.

REFRACTIVE SURGERY…..WHY BOTHER?  For patients who are significantly myopic it will change their life  Patients look to their Optometrist for information and guidance about refractive surgery  Refractive surgery is an “event” in the patients visual life – like their first glasses or contact lenses – or their first bifocal  You are their eye doctor – for their visual life and it is our responsibility to advise them on all the options and choices that they will face in their visual life Why wouldn’t you ask?

THE SCIENCE NOVEMBER 2014 – FDA PRESENTS COMPREHENSIVE STUDY ON LASIK QUALITY OF LIFE PROJECT “The Refractive Surgery Alliance (RSA) is grateful to the FDA, NIH, and DOD for conducting this comprehensive study,” said Dr. Kugler. “The data confirm what patients and their refractive surgeons have known for many years: That Lasik is a safe and effective treatment for the correction of myopia, hyperopia and astigmatism.”* *visit horizonlaser.com for full report

MEET THE TEAM Clinic Directors: Kim Nguyen Nesteroff and Susan Siroski HLVC Surgeons: Dr. Ronan Conlon, Dr. Ravi Nrusimhadevara, Dr. Jeffrey Judelson, Dr. Renatta Varma HLVC OD’s: Dr. Carmela Giocoli, Dr. Angie Machmer Patient Care Consultants: Heidi Friesen, Lisa Bautista, Jocelyn Underhill, TJ Parlee Nurses: Terri Nixey, Cheryl Wasserman, Amanda Ringdal, Chelsey Spence, Brittany McNally, Penny Ostrom, Marge Materi, Paula Tiefenbach, Terrilynn Roedelbronn, Terry Masse

PREVIEW CONSULTATION Timeline of “ curious” to “ serious “ is about 22 months. Preview consultations can be done to pre-screen patients and to find out if refractive surgery is an option, prior to the patient being out of their contact lenses. Testing will be done and the Horizon Optometrist will look it over to give an opinion if the patient is a candidate or not. If the patient is a candidate, they will be advised to book an eye exam with their optometrist for a full eye exam, if required and pre-op. You will receive a note from HLVC to inform you of any testing results and opinions given by the HLVC Optometrist.

PREVIEW CONSULTATION

REFERRAL PROTOCOL Patients will require a recent full examination; not necessary to have patient out of contacts at this point. Dry eyes should be treated and controlled before assessment at Horizon. Referral forms should be completely filled out and sent to Horizon. 1. You are not REQUIRED to have a separate visit for a laser consult. 2. You are not REQUIRED to perform a cycloplegic refraction in all cases. Cycloplegic refraction is still required for all hyperopes, however. The patient will be booked for a laser consult with the Horizon OD after the patient has been out of contact lenses for the appropriate time. Charts are reviewed by the surgeon and they will make the final determination of patient suitability. The Horizon Optometrist will inform the referring doctor of the outcome of the assessment and recommended treatment. Horizon will notify the referring doctor if a candidate is rejected. The HLVC consultation process is smoother for the patient and for referring doctors. This does not preclude you from doing any testing that you feel necessary for your screening and counselling of the patients, including the cycloplegic – use your clinical judgment to determine what is necessary to determine if a patient is a good candidate.

OPEN OR SPECIFIC REFERRAL In some instances you will want to refer your patient to a particular surgeon. Patient preference. Open referrals allow HLVC staff to direct the patient to the next opening in the surgery slate. Patients may have surgery sooner.

PROCEDURES From your information, the testing at HLVC and the examination by the HLVC OD, the correct treatment procedure will be recommended for your patient. LASIK or PRK, Xtra or KAMRA Vision Other Procedures done at Horizon by the surgeons: Corneal Crosslinking PTK Post-Cataract Corrections

ROLES IN THE PROCESS Referring OD’s HLVC OD’s Surgeons

POST-OP PROTOCOL Your patient will normally see one of the HLVC OD’s or surgeons on Day one following iLASIK, or on Day 3 – 4 following PRK. They will be sent home with the drops they need to use and careful instructions. A one week appointment will be set up for you to see your patient.

Post-op forms will be sent electronically to your office for your use. Fill in the form completely and fax back to Horizon. If there are questions to the surgeon, the staff at HLVC will direct the form to the surgeon and then send the answer back to you. Patients should be seen for 1 week, 1, 3, 6 and 12 month follow-up. Driver’s license restrictions should be removed as soon as patient is within parameters; HLVC does not contact SGI.
