This is the first screen you will encounter. Select Blank Database.
Insert your disk now and select the disk type (floppy or zip).
Open the folder you have for this class. Write the name of the file. Then click on Create.
Click on Table from the Object menu. Click on Create table by using wizard.
Select the option Personal. You will select from the fields listed. Click on FirstName, then click on the single arrow. Do this for each field you will use.
When you have selected all the fields you will use, click on Next.
Type in the name of the table. Select the option of wizard setting the primary key for you. Click Finish
Click on the table you have created. Then click on Design.
This is where you will be creating the input mask for the telephone numbers, birth dates, and any other dates that you want to look a specific way. Click on the HomePhone field. When you click on the field you want to apply an input mask, the Field Properties will show. Click on the three little dots next to the Input Mask line.
Click on Phone Number. This how the phone numbers will appear in the table. Click on Finish
This is the structure for the phone numbers. Do this for each phone number used and dates used. When you have finished setting the masks, close this screen. The program will ask you if you want to save the changes, click on yes.
You are now ready to create your form for input of data. Click on Form. Then click on Create form by using wizard.
You will be using the table you created. You will use all of the fields that you selected for the table. Click on the double arrow.
All of the fields will be moved at the same time. They are now in your form. Click on Next
You will see options on how you want your form to look. These are not required. Write the name of form. Click on Finish.
You are now ready to enter data into the form. You will enter one record at a time. When have entered all the appropriate data for each record, hit the Enter button to go to the next record. Letting the program set the primary key for you, it will automatically number each record for you. You do not have to write this information. When you have entered 10 records, close the form.

You are now going to create a query. Select Query. Click on Create query by using wizard You are using the table you created.
Select FirstName, then click on the single arrow. Do this for LastName and Birthdate. Click Next
Write the name of the query. Select the option Open the query to view information Click Finish
This is your query.
You are now going to create a report. Select Report, then click on Create report using wizard. Select the query you created, then click on the double arrow.
Click Next
