The CICC Discovery Tool and Referral System The Tool Includes Two Types of Questionnaires 11 Age-Specific Questionnaires Concerning Child Development A Questionnaire About Those Early Life Experiences or Risk Factors That Predispose Children to Having Disabilities or Other Special Needs. DESC3

The CICC Discovery Tool and Referral System If The Child Was Premature The Discovery Tool Adjusts for Prematurity Definition: A baby born before 37 weeks gestation (the time from conception to birth) is considered premature; when born before 32 weeks, a child is considered significantly premature. Adjustment for Prematurity is made automatically online by the Discovery Tool. DESC5
