THE CHECK LIST  Convey clearly to the respondents the purpose of the survey  Ensure Confidentiality of data  If in doubt, repeat the question and make sure you are clear about the response  Write down the open ended responses legibly  Invest in time – Don’t rush through the questions  At the end of the survey, remember to THANK the respondents

E.g 0930, 1415 __________________________________________________ DRINKING WATER  1.2 – Multiple choice  1.3 – Only one response to be noted  1.4 – ask only to those using sources outside the house  1.7 – Record the age & gender  1.8a – Record average monthly payments SANITATION SERVICES  2.3 –2.8: Ask only those using sources outside the house  2.12a – Narrow down the recall period to the last 1 year

HEALTH SERVICES  3.2 – Question is relevant for the respondent or any other member of the family. Only one option should be recorded  3.4 – Record the response in both Kms/m and also if possible the time  3.7 & 3.8 – Multiple responses possible  3.10 – The reference period should be the last 1 year BASIC EDUCATION  4.2 – Try to record the gender of those not attending the school  : Reference source is the eldest school going child. If possible, try to record the gender of the eldest child
